ABOUT IHC
In the last year, continued dedication from our supporters has enabled the Italian Home for Children to:
  • Serve 402 children in crisis due to circumstances beyond their control.
  • Place almost 80% of our discharged children with a family.
  • Implement two new community-based programs which currently work with over 65 children in their homes and communities.
  • Enroll 17 children in our afterschool and summer camp programs.

NOTICE OF PRIVACY PRACTICES CLIENT RIGHTS & RESPONSIBILITIES

ITALIAN HOME FOR CHILDREN'S PRIVACY PRACTICES:

The Italian Home For Children takes the privacy of our children and families seriously. This Notice of Privacy Practices is intended to give you a broad understanding of how information pertaining to you and your child is shared and protected and what your rights and responsibilities are as a client of our agency.

Information compiled at the Italian Home For Children regarding a child and family will be shared as appropriate with the child, family, legal guardian and the entity purchasing the services. This information may also be shared with other professionals clinically involved in the case upon written permission of the client served or legal guardian and/or in a manner consistent with court orders or state and federal statutory requirements.

YOU HAVE THE RIGHT:

  • To personal privacy. No audio/visual recordings, one-way mirrors, videos or photographs will be used without the child and his/her family being advised as to their use and giving permission in writing.
  • Upon request, to obtain from the facility in charge of your care the name of the clinician, physician and/or other person responsible for your care or the coordination of your care.
  • To confidentiality of all records and communications to the extent provided by law.
  • Upon request, to obtain an explanation as to the relationship, if any, of the agency to any other agency or educational institution in so far as said relationship relates to your care or treatment.
  • To obtain from the person designated by the agency a copy of this notice and any rules or regulations of the agency that apply to your conduct as a client.
  • Upon request, to receive from a person designated by the agency any information which the agency has available relative to financial assistance.
  • Upon written request, to inspect specifically identified areas of your records and to receive a copy. The fee for the copy shall be determined by the rate of copying expenses.
  • To refuse to be examined, observed, or treated by students or any other clinic staff without jeopardizing access to psychiatric, psychological, or other clinical care.
  • To refuse to serve as a research subject and to refuse any care or examination when the primary purpose is educational or informational rather than therapeutic. Written permission of the legal guardian and the informed consent of the child are prerequisites to any such activity.
  • To privacy during clinic treatment or other rendering of care.

YOU HAVE THE RESPONSIBILITY:

  • To share information with us pertinent to your child's treatment and education.
  • To understand your role in your child's treatment plan.
  • If you refuse treatment, to accept full responsibility for your decision and the consequences to your child's care.
  • To follow Italian Home For Children's rules and regulations.
  • To respect the privacy of other children and their families involved with you and your child at IHC.
  • To express your concerns to Italian Home caregivers in a respectful manner.

The Italian Home for Children has the right to change and revise this notice as necessary.
