Record Request Policy
All record requests must be received in writing, addressed to the program providing the services and indicating the dates of service. Clients requesting copies of their record must fill out the Italian Home for Children Client Record Request form and have their signature notarized. Requests from providers must be submitted on letterhead with a HIPPA-compliant release of information attached to the request. Requests from Government entities must be submitted in writing but do not require any additional documentation. Fees may be charged for copying, labor and mailing.
